FIRST TIME PASSPORT APPLICANT
You must apply in person if:
- You are applying for your first U.S. passport
- You are under age 16
- Your previous U.S. passport was issued when you were under age 16
- Your previous U.S. passport was lost, stolen, or damaged
- Your previous U.S. passport was issued more than 15 years ago
- Your name has changed since your U.S. passport was issued and you are unable to legally document your name change
Steps if applying for the first time:
1. Fill Out Form DS-11: Application For A U.S. Passport
2. Submit Completed Form DS-11 In Person
3. Submit Evidence of U.S. Citizenship
4. Present Identification
5. Submit a Photocopy of the Identification Document(s) Presented (Step 4)
6. Pay the Applicable Fee
7. Provide One Passport Photo

RENEWING YOU PASSPORT
*If your passport will be 6 months or less from the expiration date when you are scheduled to travel, you must renew it to entry many countries.*
You can renew your passport by mail if your current passport:
- Is undamaged and can be submitted with your application
- Was issued when you were age 16 or older
- Was issued within the last 15 years
Was issued in your current name or you can legally document your name change with original or certified copy of your marriage certificate or the government-issued document evidencing your legal name change
Steps to renew:
- Complete and print form DS-82
- Sign and Date page 1 of form
- Mail Form DS-82 in a padded envelope to the address listed on the form, along with:
- Your most recent U.S. Passport (book or card)
- One Passport Photo that is stapled to the application within the space provided using four vertical staples, one in each corner
- Current Passport Fees
- A marriage certificate or court order (only if your current name is different from what was recorded in your most recent U.S. passport)

Make sure to read the requirements on the website thoroughly before turning in your form to insure you have all accompanying documents, two checks, and wait to sign form until turning it in.
