Plant Analysis Submission Forms

If you are from out-of-state and do not have access to plant analysis kits, you may submit your sample to the laboratory with one of the following forms (pdf format). Samples will be analyzed for nitrogen, phosphorus, potassium, calcium, magnesium, manganese, iron, copper, boron, and zinc. Payment of $24.00 per sample must accompany your sample(s). (Please use these submission forms for crop recommendations.)

Important: Please follow all instructions for taking a plant sample (see link below) and package sample in paper bags, not plastic, prior to sending to the laboratory.
